Volume table of contents (VTOC) read error on device &3. (C R)

Message Long Text :

Cause . . . . . : The device was not able to read the volume or file labels from the diskette while attempting to process file &1 in library &2 in device &3.
Recovery . . . : Type C to cancel processing, or type R to try the read operation again.
Possible choices for replying to message . . . . . . . . . . . . . . . :
C -- Cancel the diskette file processing.
R -- Retry the operation.
Technical description . . . . . . . . : The diskette is defective, not prepared correctly, or otherwise not readable. Insert a different diskette in &3 to resume processing, or prepare the diskette (INZDKT command). Error Log ID &22.
