Permanent I/O error on volume &9 in device &4.

Message Long Text :

Cause . . . . . : The diskette in device &4 used for file &2 in library &3 contains permanent input/output (I/O) errors that device &4 cannot process.
Recovery . . . : Do one of the following and try the request again:
-- For output files, use another diskette for output files.
-- For input files, create the file again using another diskette, if the error occurs again.
-- Ensure the correct diskette is inserted in the diskette unit.
Technical description . . . . . . . . : Error Log ID &22.
