ORA-01562: 'failed to extend rollback segment'

Symptom

Oracle error message ORA-01562 'Failed to extend rollback segment' +
ORA-01547 'Failed to allocate extent of size ... in tablespace ...'

Cause

Possible causes:
- Rollback segments are dimensioned too small.
- Long-running transactions are blocking the rollback segments.
- Rollback segments are not dimensioned equally, so that Transactions
are sometimes completed and sometimes not.
- Extents are not equally large.
- Number of rollback segments is too small.

Solution

Rule: - Create all rollback segments with the same extent size
and the same number of extents.
- MINEXTENTS = MAXEXTENTS for all rollback segments
- Increase number of rollback segments.
