Title

Forms for Employees State Insurance (HINCESI0)

Purpose

This report prints the following statutory forms for Employee State Insurance depending on the contribution period.

If the contribution period is upto March 2007, then the report prints:

  • Form 6
  • Form 7

If the contribution period is after March 2007, then the report prints:

  • Form 5
  • Form 6

Note

ESIC authorities have introduced ESI monthly in the year 2011 to file employee's monthly details.

Integration

This report reads the Results Table (RT) and the Other Statutory Deductions (0588) infotype with subtype ESI (0001) for details to be printed on the forms.

Prerequisites

  • Other Statutory Deductions (0588) infotype with subtype ESI (0001) records must exist for the selected employees.
  • ESI wage types must exist for the selected employees in the RT, for the selected contribution period.

  • These forms are printed at the end of each contribution period.

Activities

  1. Choose Human Resources -> Payroll -> Asia/Pacific -> India -> Subsequent Activities -> Other periods -> Legal Reports -> Employee State Insurance -> Forms for Employee State Insurance
The Forms for Employee State Insurance Report screen appears.
  1. Enter the employee selection criteria.
  2. Enter the contribution period. You must enter one of the following contribution periods for a financial year:
    1. April 1 - September 30, or
    2. October 1 - March 31
Note
If the contribution period is after April 1, 2007:
- The fields Personnel No. of Person Resp., Report Date, Report Place and Challan Details pushbutton in User Parameterssection become editable. Now you can enter the details in the corresponding fields.
- The system highlights the relevent forms in the Output format section.
If the contribution period is before April 1, 2007:
- The fields Personnel No. of Person Resp., Report Date, Report Placeand Challan Details pushbutton in User Parameterssection become non-editable.
- The system highlights the relevent forms in the Output format section.
  1. Select the option for Days wages paid Cal/Work indicator. Depending on your selection, the system will calculate the number of ESI working days in the month as calendar days or working days.
  2. Select the option for Unpaid absence days Cal/Work indicator. Depending on your selection, the system will calculate the number of unpaid absences in the month as calendar days or working days.
The system subtracts the number of unpaid absences from the number of ESI working days in the month to determine the number of days for which wages are paid to the employee.
  1. Select the Personnel No. of Person Resp.checkbox.
This person is responsible for submitting the form 5 to the government authorities.
  1. Choose Challan Details to fill the required challan details. When you choose this, the system displays the Challan Details screen. On this screen, you must enter the following:
  • Month

  • Date of Challan

  • Amount

  • Name of the Bank and Branch

Choose Save Challan Details.
Note
If a company submits the challan to only one bank, then you can enter only six rows in the challan details screen.
The screen can accept and print more than 6 rows only when a company is submitting the challan to more than one bank for the same month.
  1. Select the report you want to print.
  2. Select Print Address of Personnel Area indicator if you want the system to print, under the Name and Address of the Factory or Establishment section of the forms, the name and address that corresponds to the Personnel Area to which the employee belongs.
  3. Select Customer Layout, if you want to print the report in the layout that you have created.
  4. Choose Program -> Execute.
The ESI Report output screen appears.
If you have chosen contribution period after April 1, 2007 then the system displays Form 6 or Form 7 details. In this case, you are able to print the forms.
If you have chosen contribution period before April 1, 2007 then the system displays Form 5 or Form 6 details. In this case:
  • If you choose Form 5, you are able to print the form.

  • If you choose Form 6, you are able to download the form in Excel sheet format.

  1. Select Print Form
