HOMPOH0 - Form 17

Purpose

This report generates a Form 17, which contains the following details:

• Social insurance details of the employee who is injured due to occupational hazard

• Employer's social insurance details

• Injury details such as place of injury, date and time of injury, kind of injury, summary of accident

• Employee's salary on the day of injury

• Previous accident details, if any

You can submit this form to the Public Authority of Social Insurance (PASI) for claims.

Selection

• Specify the reporting period for which you want to execute this report.

• Specify the delection criteria.

• In the Report Exclusions section, select the Evaluate Excluded Employeescheckbox, if you want the report to process the details of employee's excluded from legal reporting.

• Specify the name of the form that this report must generate in the Form Name field. In the standard solution, by default, the system uses the form, HOMPOH0.

Standard Variants

Output

On the output screen of this report, the system displays the list of employees for whom data was processed. You can choose a specified record from the list displayed and select the Print PDF button to generate Form 17 for the employee. You can also use the Send PFD as E-mail to Employee, if you want to send the Form17 as an e-mail attachment to the employee.
