Headers for FORM/MODULE are not always created
Tip-No.: 1659

Symptom

Pretty printer does not always insert headers for FORM or MODULE.

Cause

There is already a comment before the FORM or MODULE, which the
Pretty printer must recognise as the header.

Solution

No change possible, because otherwise repeated Pretty printing
would always insert a new header.
