Create Preregistration for Surgery and Treatment

Use

If you use IS-H*MED, you can preregister patients for an inpatient admission and for a surgery or treatment. You can use both of these situations of the preregistration independently or together.

In addition to the function Create Preregistration for Inpatient Admission, the function Create Preregistration provides the following options for preregistering patients for a surgery or treatment:

Dialog Structure

A service facility supplies all preregistration types that enable you to preregister a surgery or a treatment. Reason: The preregistration of a surgery or treatment takes place based on the services from the service range of the service facility. If you need to preregister the patient for a surgery or treatment, you must select a service facility and the desired preregistration type in the Treating (Attending) Org. Unit field.

  • Srg/Treatment, Admission Tab Page
  • The Srg/Treatment, Admission tab page replaces the Admission tab page if you select a preregistration type that enables you to preregister services.

  • After selecting the preregistration type, it will contain possible services that the preregistration type supplies to the user. As soon as the user has selected one or more services, the display will switch to the entry display where the selected services will appear. Also, now the Planning group is visible, which includes the case type, the admission appointment, the surgery/treatment appointment, and the operating surgeon.

  • You can use the Case Type to specify whether to execute the surgery or treatment as an inpatient, outpatient or a day patient one. If you select inpatient or day patient, then both the Admission Appointment and Care Unit fields will appear. For outpatient cases, it is not necessary to schedule an admission appointment. The visit appointment scheduled for the preregistered services will appear on the scheduled day in the OR schedule and in the work station of the service facility.

  • The Admission Type specified depends on the case type and is optional for entry if the time of the preregistration is already known.

  • If you do not immediately schedule an admission appointment or a surgery/treatment appointment due to the number of preregistered patients or due to organizational processes, you can still enter a Desired Month to the right of the group. You can also sort the preregistrations that do not have a surgery/treatment appointment or an admission appointment according to this desired month in the preregistration list.

  • If there is already a Treatment Categoryat the time of the preregistration, you can enter this as well.

  • You can use the treatment category to set the Private Patient indicator, if a case classification is also assigned to the treatment category entered in customizing. Here, the case classification will indicate private patients.

  • The system will display the Operating Surgeon field if the selected preregistration type has the Srg indicator. In the Operating Surgeon field, you can enter all employees allowed in the team entry as an operating surgeon. If it is necessary to enter more team members like an assistant or a second surgeon when creating the preregistration, you can call the entry dialog for the surgery team by using Enter Team.
    If you would like to enter a surgeon in the preregistration who is not an employee of your institution, then you can use the External Employee function on the left next to the entry field.

  • The surgery/treatment appointment is an appointment scheduled for the preregistered services. You can enter this manually or determine it by using the function Find Surgery/Treatment Appointment. Whether you schedule the surgery/treatment appointments day-specifically or time-specifically depends on the OU-related parameter N1VKGPLMOD. You can use this parameter setting to make the entry field for the time visible and to call the day-specific planning function or the planning grid for finding appointments.

  • In addition to the srg/treatment appointment, you can also enter the Admission Appointment. Depending on the setting of the OU-related parameter N1VKGPLMOD, you can enter the admission appointment with or without the Time. Since inpatient admission appointments are scheduled many times day-specifically and the patients are routinely at admission in the morning, the "Find Admission Appointment" pushbutton will branch you into the day-specific scheduling function independent of a parameter setting.

  • If you schedule an admission appointment, you also need to enter a Care Unit for the admission time, where the patient admission will take place.

  • Referral or Transfer Tab Page
  • Depending on the case type entered, the Create Preregistration function will supply a Referral tab page for inpatient and day patient cases, as an alternative to the Transfer tab page for outpatient cases. If you do not specify a case type, the Referral or Transfer tab page will not be available.

  • The Referralor Transfer tab page supplies entries for the referring or transferring physician, for the family physician, and the referring or transferring hospital. You can transfer this data with the preregistration into the admission dialog when admitting the patient.

  • If you enter the preregistration for a patient, who already exists in the patient master, the system will propose a referring physician and a family physician from the master data of the patient. This only takes place if the user branches to the Referralor Transfer tab page.

  • Effects on Existing Data

Private Patient Indicator:

  • For preregistrations created up to Release 4.62, you can manually set the Private Patient indicator without a link to the treatment category. You can manually reset a Private Patient indicator that was created up to and including Release 4.62, but you can only set this by entering a treatment category.
  • Automatic conversion is not possible since the system cannot uniquely determine a treatment category from the indicator Private Patient.

Effects on Data Transfer

Effects on System Administration

The existing Authorization Object N_VKG_FCT is still supported. The system will then always check authorizations for this object, if the user creates a preregistration for a service facility.

The system will check authorizations for this object N_VKF_FCT (see Create Preregistration for Inpatient Admission), if the user preregisters a patient for a department.

Effects on Customizing

The following settings are required for the Create Preregistration dialog:

  • The selection list with all available preregistration types and services is integrated into the entry dialog. If you would like to have a pre-switched dialog box that displays the available preregistration types and services, you can set this using the system parameter N1VKGPA (institution-related, time-independent). You can also access more detailed information for this in the Documentation for the System Parameter N1VKGTPA.
  • OU-Related Parameter N1VKGPLMOD: determines whether to schedule the surgery/treatment appointment and the admission appointment day-specifically or time-specifically. You need to maintain the parameter for any org. unit supplying the preregistration type. The default setting for the parameter N1VKGPLMOD is Day-Specific Planning. The Time field of the admission appointment will be displayed depending on the settings of the parameter. The parameter applies to both appointments.
  • For the surgeon entered, the system will require a task to which the employee of the surgery team should be assigned. You need to store this task in N1VKGAGOP. You must always maintain the parameter N1VKGAGOP for any service facility supplying the preregistration type. If you use the same tasks for all operating service facilities in your institution, then you only need to enter parameter N1VKGAGOP one time for the org. unit "*".
