Status Management for Medical Basic Documentation (New Concept)

Use

The status management for Medical Basic Documentation has been improved in the following areas:

  • Authorization check
  • Billing blocks

To make these improvements, it was necessary to replace the predefined system statuses with "user statuses".

With IS-H Release 4.63B, SAP introduces a new concept for managing the status of medical basic documentation. The main features of this concept are:

  • Definition of customer-specific status profiles containing any number of user statuses.
  • You can use the designation of your choosing for these statuses.

  • You can define your own sequence for these user statuses.

  • Provision of business transactions that can be allowed or disallowed in relation to a status.
SAP currently provides the following four business transactions:
  • Bill Case

  • Group DRG

  • Change Diagnoses

  • Change Procedures

  • Change documents are also recorded.
  • The dialog for changing the status is now accommodated by a list box that contains statuses allowed for the current user and case.

You can change the status in the transactions:

  • NP37: IS-H: Create Surgery (Visit)
  • NP47: IS-H: Maintain Case-Related Procedures
  • NP61: IS-H: Maintain Diagnoses by Case/N222: IS-H*MED: Maintain Diagnoses by Case
  • NPDRG2: IS-H: Maintain DRG Data

Effects on Billing

You can use the billing block and a user status to prevent a case from being billed.

If you have set the Billing Block indicator, the system issues the message N8 367 Case & is blocked for billing (live billing not possible) during the billing run, as in previous releases.

If the case to be billed has a user status that prevents the business transaction "Billing", the system issues the new message N6 485 Business transaction "Bill Case" is not supported during the billing run. If you are running the billing program in live mode, the system issues this message as an error message, and terminates billing. For test billing, you can customize this message along the same lines as the message N8 367 in IS-H Message Control. The system outputs a warning message by default in the case of a test billing. You can modify the system behavior by maintaining the relevant entry for this message (Function A000, AppAr N6, MsgNo 485, Default Type W) in the IS-H Implementation Guide by choosing SAP Healthcare - Industry-Specific Components for Hospitals -> Basic Settings -> System Parameters -> Message Control -> Maintain Messages.

Effects on Existing Data

Cases previously assigned a status for their medical basic documentation using the BAdI ISH_INIT_MD_STATUS, are automatically converted into a standard status profile during the release upgrade (report RNUX_463B_MD_STATUS). You can enhance the standard status profile using the IMG activities. The individual statuses can be modified but not deleted.

Note that the statuses of the standard status profile are linked to the business transaction "Bill Case" in the standard shipment. If you wish to prevent billing in relation to the status of the medical basic documentation - as in the previous status management concept - you can do so in the IMG activity for maintaining the status profile.

  • in the view type Coding Analyses
  • in the report for grouping multiple cases (RNUDRG0A).
