Allow adding new lines in almost anywhere in the form at run time

Description

In the previous release, user can only add, modify, delete lines under total lines. This functionality, although useful at emergency cases, is not flexible enough for day-to-day usage.

Starting this release, user can add lines above any input line, added line, or, total line. In addition, an explicit "Modify" button is added to avoid confusion.
