ISHMED_PKMS_01 - System Adjustments to Be Made

This document concerns nursing complex procedure scores (German: PKMS), which are relevant for the country version Germany only. In the nursing process documentation nursing complex procedure scores (PKMS) can be automatically derived. In order to achieve this, you must connect the entered nursing procedures, nursing diagnoses, nursing problems and nursing goals with corresponding PKMS interventions. Based on these connections, the system evaluates the corresponding PKMS values per day and case, either in a dialog or using a report. Furthermore, within the determination of levels of care (NSR) for the highly complex treatment of patients, the corresponding level of care A4 can also be derived.

To integrate PKMS derivation, you must execute the following parameterization and system adjustments:

1. Create a basic catalog of the "PKMG - PKMS Reasons" catalog type

You must create a basic catalog of the "PKMG - PKMS Reasons" catalog type.
You either create this manually (see Manage Basic Catalogs) or import the supplied catalogs according to the instructions in note number 1755070.

1. Create a basic catalog of the "PKMI - PKMS Procedures" catalog type

You must create a basic catalog of the "PKMI - PKMS Procedures" catalog type.
You either create this manually (see Manage Basic Catalogs) or import the supplied catalogs according to the instructions in note number 1755070.

1. Connect PKMS Procedures with PKMS Reasons

You must connect each PKMS procedure with the corresponding PKMS reasons. It is therefore essential that you adhere precisely to the sequence (first create the PKMS reason catalog, then create the PKMS procedure catalog).
The connections are alreasy implemented in the catalog supplied in the standard delivery.

1. Connection of Nursing Diagnoses, Nursing Problems and Nursing Goals

You must connect the used nursing diagnoses, nursing problems and nursing goals with entries from the basic catalog of the "PKMG - PKMS Reasons" catalog type with the catalog name "PKMS Reasons" (see Connect Goals, Problems and Diagnoses with PKMS Reasons).

1. Connection of Nursing Procedures

You must connect the used nursing procedures with entries from the basic catalog of the "PKMI - PKMS Procedures" catalog type with the catalog name "PKMS Interventions" (see Connect Nursing Procedures with PKMS Procedures).

1. Service Master Data Enhancement

In the service master data (transaction NT01) you must create a service record for the service catalog 1 for each nursing category of PKMS entry for example, personal hygiene, movement, (see Check Service Master Data).

1. Procedure Rule for Deriving OPS Codes

The procedure rule for deriving the OPS code must exist for the PKMS value. According to SAP preset values this is procedure rule P10. Check whether the procedure rule P10 is installed. If this is not the case, execute the corresponding parameterization according to SAP note number 1517181 from October 2010.
You should also refer to SAP note number 1642197 regarding PKMS version for 2012.

1. Consideration of NSR Level of Care A4

You must also make adjustments for the generation of level of care NSR A4. In Customizing under Industry-Specific Components for Hospitals -> Medical/Nursing Documentation -> Define Levels for Nursing Acuity Classification check whether the new A4 categories exist for the adult, adolescent and children patient groups. If these levels do not yet exist, enhance the system configuration as stipulated by SAP. You should note that the level of care A4 does not exist for babies.

1. Activation of Relevant Business Add-Ins (BAdIs)

You must activate the BAdIs necessary for PKMS derivation.
